For the past eight years, I have worked in healthcare IT. Before joining Cerner, now Oracle Health. I spent years in the telecom and ISP world as a technology architect. Back then, APIs, service orchestration, and interoperability were not “strategic initiatives”; they were simply how systems survived at scale.
If services could not talk to each other, customers felt it immediately and the system failed.
Moving into healthcare was eye-opening. The stakes were fundamentally different. In most industries, technology failures impact revenue, customer satisfaction, or brand reputation. In healthcare, failures affect patient safety, quality of care, and human life. That single difference explains why healthcare technology evolves more cautiously, and why shortcuts are far more dangerous.
It also explains why interoperability remains one of the hardest, yet most critical, problems to solve.
Interoperability: The Unfinished Foundation of Healthcare IT
Healthcare interoperability is often discussed as a technical challenge, but in practice, it is a systemic one. It touches data standards, governance, clinical workflows, legal frameworks, and human behavior all at once.
On paper, we have made progress. Standards such as FHIR exist. APIs are widely discussed. Cloud platforms promise flexibility and scale. Yet on the ground, clinicians still struggle with fragmented patient records, duplicated documentation, and systems that do not reflect how care is actually delivered.
The question is not whether interoperability matters.
The real question is why it remains so difficult despite years of effort.
When Data Speaks Different Languages
One of the earliest challenges I encountered in healthcare was the variability of data itself. Different systems capture the same clinical concept in different ways, using different formats, terminologies, and structures.
This is not just a technical inconvenience. When data is inconsistent, clinical meaning is lost. Decision support becomes unreliable. Analytics becomes questionable. AI models become dangerous.
The root of the problem is historical. Healthcare systems evolved independently, often optimized for local workflows rather than ecosystem wide data exchange. Proprietary formats persisted, and standards adoption remained uneven.
Progress starts with standardization not perfection, but consistency. Modern standards such as FHIR provide a common language, but only when they are implemented thoughtfully and paired with shared clinical terminology. Without this foundation, advanced analytics and AI rest on unstable ground.
Trust, Privacy, and the Cost of Getting It Wrong
Unlike many industries, healthcare does not have the luxury of “moving fast and breaking things.” Patient data is deeply personal, and privacy regulations exist for good reason.
Security concerns often slow interoperability initiatives, but in reality, security and interoperability are not opposites. Poorly designed, fragmented systems are often less secure, not more.
Building trust requires security by design: encryption, access control, auditing, and clear accountability. It also requires educating healthcare professionals so that security becomes an enabler of care not an obstacle.
Without trust, data sharing stalls. And without data sharing, digital healthcare cannot scale.
Fragmentation: A Daily Reality for Clinicians
Most healthcare organizations did not design their digital ecosystems; they accumulated them. Over time, different EHRs, departmental systems, and medical devices were added to solve immediate problems.
The result is fragmentation.
Clinicians feel this every day. Information exists, but not where it is needed, not when it is needed, and not in a form that supports decision-making. Interoperability is often discussed at an architectural level, but its absence is experienced at the bedside.
The solution is not ripping and replacing systems overnight. It is adopting API-first architectures, standardized integration layers, and incremental modernization strategies that respect both clinical workflows and operational realities.
Data Ownership, Governance, and the Question No One Likes to Answer
As interoperability improves, uncomfortable questions surface:
Who owns the data? Who can access it? Who is accountable when it is misused?
These are not technical questions, yet they directly impact technical design. Without clear data governance frameworks, interoperability initiatives slow down or fail entirely.
Healthcare organizations must define ownership, access rights, and usage policies clearly and transparently. Governance is not bureaucracy, it is what allows data to move safely, ethically, and at scale as we are going to see this in the usage of AI in the future.
The Human Side of Change
Even the best-designed systems fail if the people using them do not trust them.
Healthcare professionals are rightly cautious about new technology. Many have experienced systems that promised efficiency but delivered frustration. Resistance to change is often framed as a cultural problem, but in reality, it is usually a response to past failures.
Successful interoperability initiatives involve clinicians early, respect existing workflows, and demonstrate measurable improvements in patient care and daily work. Trust is built through results, not presentations.
Vendor Lock-In and the Limits of Closed Ecosystems
Vendor lock-in remains one of the quiet blockers of interoperability. Long-term contracts, proprietary data models, and limited portability restrict innovation and collaboration.
True interoperability requires open standards, transparent APIs, and contractual flexibility. Data must remain accessible to the organizations and patients it represents, not trapped inside platforms.
Where AI Fits—and Where It Must Wait
AI is often presented as the next revolution in healthcare, but its success depends entirely on what comes before it.
The safest and most impactful early use of AI is not autonomous decision-making, but operational relief:
- Reducing administrative burden
- Improving documentation accuracy
- Supporting clinical decision-making without overriding it
AI systems learn from data. If that data is fragmented, inconsistent, or poorly governed, AI will amplify existing problems rather than solve them.
Interoperability is not optional for AI it is a prerequisite.
Conclusion: Technology That Learns to Heal
Healthcare interoperability is not just an IT problem. It is a patient safety issue, a clinician experience issue, and a prerequisite for responsible AI.
Before healthcare can fully benefit from artificial intelligence, its systems must first learn to communicate, to trust, and to respect the realities of care delivery.
